streamlined application form and the Adobe Acrobat Reader you can
complete the process at your PC in just a few minutes. To apply, just
do the following:
(1) Click on the button
below to open the application form in PDF format. To move around the
form just press your TAB key. Phone numbers and social security numbers
should be entered numerically, with no punctuation, while dates must be
formatted
as XX/XX/XXXX. Once completed, just print, sign, and bring to the
credit union office with your picture ID.
(2) If you have difficulty
with the above method, we recommend that you try this. Right click this
link, select "Save Target As" and place it on your Desktop. You can
then open it by double-clicking the mbrapp.pdf icon on your Desktop.
Once the Acrobat Reader opens up the document, complete, print, and
deliver the form as directed above.

INFORMATION ABOUT PROCEDURES FOR OPENING A NEW ACCOUNT
To help the government
fight the funding of terrorism and money laundering activities, federal
law requires all financial institutions to obtain, verify, and record
information that identifies each person who opens an account. What this
means for you: When you open an account, we will ask for your name,
address, date of birth, and other information that will allow us to
identify you. We may also ask to see your driver's license or other
identifying documents.
